ABSTRACT

Objective:To search for valuable laboratory indexes of early diagnosis of anastomotic leakage after anterior resection for rectal carcinoma.Methods:From Sep 2017 to Jan 2019, 128 patients with colorectal cancer underwent anterior rectal resection at the Department of Colorectal & Anal Surgery, the 940 Hospital of the Joint Logistics Support Force.Results:Anastomotic leakage occurred in 16 of 128 patients (12.5%). Definite diagnosis of anastomotic leakage was made on between 2nd and 9th day, postopera tively averagign (6.13±2.00) days. Tumor location was a risk factor for anastomotic leakage with the incidence significantly lower when the distance from the lower edge of the tumor to the anal margin >7 cm than when the distance ≤7 cm ( χ 2=6.022, P=0.014). The percentage of increase in peripheral blood leukocytes, neutrophils, serum interleukin-6, C-reactive protein and procalcitonin in patients 3-5 days after surgery significantly related to the occurrence of anastomotic leakage (all P<0.05). The area under the working characteristic curve of the subjects with the percentage of C-reactive protein, procalcitonin, interleukin-6, leukocytes and neutrophils from the 3rd to the 5th day after operation was greater than 0.5. Conclusion:C-reactive protein, procalcitonin, interleukin-6, leukocyte and neutrophil percentage are risk factors predicting anastomotic leakage after anterior resection of rectal cancer.

ABSTRACT

Objective:To investigate the effect of cisplatin on the expression of programmed death factor ligand 1 (PD-L1) in SGC-7901 cells of gastric cancer and the regulation of lymphocyte on the proliferation of gastric cancer cell.Methods:Flow cytometry was used to detect the expression of PD-L1 on the surface of SGC-7901 cells. After cisplatin pretreatment, SGC-7901 cells were co-incubated with the activated lymphocytes. In group B, SGC-7901 + lymphocytes were not treated with cisplatin. Cisplatin treated SGC-7901 + lymphocytes were in group C. The apoptosis of CD 4+ and CD 8+ T cells in each group was detected by flow cytometry. The dissolution rate of SGC-7901 cells was calculated by ADCC method. Results:MTT results showed that cisplatin inhibited the proliferation effect of SGC-7901 cells in a concentration-dependent manner after 48 h′ cisplatin treatment ( F = 128.35, P < 0.05). After SGC-7901 cells were treated with cisplatin for 0, 0.5, 1.0, 2.0, 4.0 mg/L, the expression of PD-L1 on the cell surface was up-regulated, and after cisplatin treatment for more than 1.0 mg/L, the expression began to decline ( F = 477.79, P<0.05). After cisplatin treatment, SGC-7901 cells were co-incubated with lymphocytes, which increased the apoptosis rate of lymphocytes ( F = 524.98 and 122.47, P<0.05). When human peripheral blood mononuclear cells and target SGC-7901 cells were incubated, cisplatin could mediate the killing activity of human peripheral blood mononuclear cells on human SGC-7901 cells, (ADCC effect), and cisplatin could weaken the killing activity ( t = 15.961, P < 0.05). Conclusions:Cisplatin may inhibit the killing activity of human peripheral mononuclear cells to SGC-7901 cells of gastric cancer and weaken the death of gastric cancer cells by inducing the expression of PD-L1 in SGC-7901 cells.

ABSTRACT

Objective To explore the efficacy and safety of greenlight photoselective vaporization of prostate (PVP) in high-risk elder patients with benign prostatic hyperplasia (BPH). Methods A total of 120 high-risk elder patients with BPH underwent PVP procedure. The operating time, blood 1oss, indwelling catheterization and operation-related complications were monitored. The variables such as international prostate symptom score (IPSS), Quality of life (QOL) score, maximum urinary flow rate (Qmax) and residual urine volume (RUV) were recorded and compared pre- and post-operatively. Results All the 120 patients had a good peri-operative condition. The mean operating time was (52. 6 ± 5.8 )min, intraoperative blood loss was (20.4 ±9.5) ml. Among the 120 cases,26 did not received postoperative catheterization. In the other 94 cases received postoperative catheterization, the mean catheterization time was (24. 5 ± 15.9) hour. IPSS and QOL scores decreased from (28.5 ± 3.8) and (5.0 ± 0.8) preoperatively to (7.2 ± 1.8) and (1.5 ± 0.6)postoperatively. Qmax increased from ( 5.2 ± 3.1 ) ml/s to ( 15.2 ± 4.3 ) ml/s, and RUV decreased from ( 118.6 ± 15.2) ml to ( 16. 5 ± 4. 6) ml. There was significant difference for these parameters before and after the operation (Ps < 0. 05 ). Conclusion The PVP is considered to be a safe, effective and minimally invasive procedure to treat BPHin high-risk elder patients. It is easy to manipulate, with advantages of shorter operating time,less blood loss, better tolerance and rapid recovery, especially for high-risk elder patients with BPH.

ABSTRACT

As the main medicinal powder for drawing out pus and removing necrotic tissue in external therapies of traditional Chinese surgery, Sheng Powder has made great contributions to the treatment of inflammatory wounds and has the unique bactericidal and decay-discharging function that can not be replaced by antibiotics. However, Sheng Powder has toxicity because it contains mercury. So far, there is no clinical research on the standards of dose and usage of Sheng Powder and there is a lack of objective and quantitative criteria for operating standards and monitoring of toxicity and side effects. Therefore, the authors choose Jiuyi Powder, one of the most commonly used Sheng Powder, to evaluate the safety of its external use, and form a standardization program for clinical implementation.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the method of adenovirus vector-mediated herpes simplex virus-thymidine kinase gene (ADV-TK)/ganciclovir(GCV) system and photodynamic therapy(PDT) for treating the oral malignant tumor.</p><p><b>METHODS</b>Ten patients who were suffering from oral malignant tumor of the different positions were selected, and injected with the hematoporphyrin derivative (HPD), irradiated by picking the corresponding laser frequency, and injected the ADV-TK gene into the tumor body and periphery. By the imaging and the hemodynamics analysis, the clinical efficacy after infusing vein with GCV was assessed.</p><p><b>RESULTS</b>After the combination method treatments, patients' tumors appeared clear shrinkage or complete extinction. There was obvious fall of the blood flow volume in the tumor body. Imaging results showed significantly differences. So it was a perfect and effective treatment.</p><p><b>CONCLUSION</b>There are many advantages to apply the ADV-TK/GCV system and PDT treatment on the oral malignant tumor, minimal side effects and greater clinical security. It is a safe and credible therapy which can be offered for curing the oral malignant tumor systematically.</p>

ABSTRACT

Objective To study and evaluate various therapeutic means for upper urinary tract calculi. Methods 3 218 cases of upper urinary tract calculi were reviewed.1 365 were pure kidney stones,651 complicated kidney stones,558 group Ⅰ ureteral stones,452 group Ⅱ ureteral stones and 192 group Ⅲ ureteral stones. Results ESWL (extracoporeal shock wave lithotripsy) has been the treatment of choice for pure kidney stones and for groupⅠ ureteral stones.Open surgery has been the best for complicated kidney stones and group Ⅱ,Ⅲ ureteral stones.1 164 patients underwent lithotomy and 1 923 ESWL.131 cases of group Ⅱ ureteral stones in the middle or lower segment of ureter were treated by ureteroscopic lithotripsy.Of the complicated kidney stones,residual stones were observed in 57 (8.8%).1.8%( n =12) of such cases underwent nephrectomy.The complete stone elimination rate in 3 months after ESWL was 81.6% whereas open operation was needed in 18.4%.Ure teroscopic lithotripsy yielded the poorest outcome,nephrectomy being undertaken afterwards in 0.8% and lithotomy in 6.1%. Conclusions ESWL,ureteroscopic lithotripsy,ballast lithotripsy or the combined use of 2 methods makes the surgical management of upper urinary tract calculi easier with less suffering to the patients.The use of double internal stent could reduce the surgical complications.Stone street after ESWL should be well taken care of and inflammation should be well controlled.Skilled technique is needed for ureteroscopic lithotripsy.

ABSTRACT

To determine the effect of propofol on myocardial contractility. Method: Twenty hamsters were allocated to receiving intravenous propofol 2.5mg?kg~(-1)(group Ⅰ) or thiopentone 4mg?kg~(-1)as control (group Ⅱ) dp/dt_(max), was determined using a catheter inserted into the left ventricle from right carotid artery. LVP (LVSP, LVDP, LVMP), CVP, HR were recorded simutaneously. Resutt: dp/dt_(max), decreased insignitieantly in group Ⅰ but signitieantly in group Ⅱ. LVP(LVSP, LVDP, LVMP) decreased, while CVP increased significantly in both groups (P0.05). Conclusion.. The influence of propofol on myocardial contractility is weaker than that of thiopenone.
